A GoHighLevel Formstack integration lets you keep the forms your team already trusts while every submission lands in your CRM as a tagged, pipeline-ready contact. It is a common request from Australian allied health, financial services and legal clients who chose Formstack years ago for its conditional logic and data-handling controls.

This guide is part of our form-tool integration series. I will walk you through the three realistic ways to connect Formstack to HighLevel, show the exact field mapping I use, break down the true monthly cost, and tell you honestly when you should retire Formstack and move to native HighLevel Forms instead.

Three ways to connect Formstack to GoHighLevel

There is no one-click Formstack app in the HighLevel marketplace, so you are choosing between a direct webhook, a middleware tool, or replacing the form altogether. Each has a different cost profile and a different failure point.

Option 1: Formstack webhook to the Inbound Webhook trigger

Formstack can post every submission to an external URL. HighLevel's Inbound Webhook workflow trigger gives you exactly that URL, so the data travels straight from Formstack into a Workflow with nothing in between.

This is my default recommendation. It is real-time, it has one less vendor holding your client data, and the per-submission cost is a fraction of a cent. If you have never used it, our walkthrough of inbound and outbound webhooks in GoHighLevel covers the trigger in more depth.

Option 2: Zapier or Make in the middle

Formstack has apps on both Zapier and Make, and so does LeadConnector (HighLevel's connector name). You pick "New Submission" in Formstack as the trigger and "Create/Update Contact" in LeadConnector as the action.

This suits teams who already run Zapier for other jobs or who need to send the same submission to several tools at once. The trade-off is a second subscription and a second place where things can silently break. We compare the approaches in our guide to GoHighLevel webhooks versus Zapier integrations.

Option 3: Replace Formstack with native HighLevel Forms

If your Formstack forms are simple lead captures, rebuilding them in HighLevel's form builder removes the integration entirely. Native forms write directly to contact fields and fire the Form Submitted trigger without premium charges.

Step-by-step: set up the Formstack webhook in HighLevel

Here is the exact sequence I use with clients. Build it in a test sub-account or on a duplicate form first, never on a live intake form.

Step 1: Create the workflow and copy the webhook URL

  • In the sub-account, go to Automation > Workflows and create a workflow from scratch.
  • Add the trigger Inbound Webhook and copy the unique URL it generates.
  • Name the workflow clearly, for example "Formstack – Initial Consult Intake".

Step 2: Add the webhook in Formstack

In Formstack, open the form and go to Settings > Emails & Actions, then add a webhook under the advanced settings. Paste the HighLevel URL, set the content type to JSON, and tick the option to send field IDs if your labels are long or likely to change.

Formstack's own webhook setup article shows the current menu path and the optional HMAC signature header. Sending field IDs rather than labels protects you when someone edits a question's wording six months later.

Step 3: Send a test submission and fetch the sample

Submit the form once with realistic dummy data in every field. Back in HighLevel, click "Fetch Sample Requests" on the trigger and select the payload you just sent.

If nothing appears, check that the form actually saved the webhook action and that you submitted the live form, not the preview.

Step 4: Map fields with Create/Update Contact

Add the Create/Update Contact action. Each Formstack field now appears as a variable you can drop into a contact field, standard or custom.

Formstack fieldHighLevel destinationNotes
Name (first/last sub-fields)First Name, Last NameEnable sub-field names in the webhook so they arrive split
EmailEmailPrimary de-duplication key
PhonePhoneAsk for +61 format to avoid SMS failures
Service of interest (dropdown)Custom field (dropdown)Option values must match exactly
Consent checkboxCustom field (checkbox) + tagStore the wording version you used
Referral sourceSource / custom fieldFeeds attribution reporting

Create your custom fields in Settings > Custom Fields before mapping. Mapping to a field that does not exist yet is the most common reason a test "works" but the contact record stays empty.

The integration is not the hard part. Deciding what each answer should trigger inside HighLevel is where the real value sits.

Tags, pipelines and the follow-up workflow

Once the contact exists, the same workflow should do the work your team would otherwise do by hand. I build it in this order so every step has clean data to act on.

Tag the source and intent

Add a source tag such as src-formstack-consult and an intent tag based on the service selected. Use If/Else branches on the custom field value. Keep your naming consistent with a documented tag strategy and naming convention, or your reports fall apart within a quarter.

Create the opportunity

Use the Create/Update Opportunity action to drop the contact into the right pipeline and stage, for example "New Enquiry". Set the opportunity value if the form captures budget or package choice.

Send the follow-up

  • An instant confirmation email via your LeadConnector or Mailgun sending domain.
  • An SMS within five minutes, only if your A2P registration (or Australian sender ID) is sorted.
  • An internal notification to the assigned team member with the key answers.
  • A task due next business day if no reply has been logged.

If the team is moving off Formstack for basic forms, the Form Submitted trigger in GoHighLevel lets you run this exact same follow-up without the webhook layer.

How to test before you go live

Run at least five test submissions covering the paths that matter. A single happy-path test hides the problems that bite in week two.

  • A brand-new contact with every field completed.
  • An existing contact submitting again, to confirm de-duplication updates rather than duplicates.
  • Optional fields left blank, to make sure no step errors on empty values.
  • Each dropdown option, to confirm every If/Else branch fires.
  • A phone number in local format, to see how HighLevel normalises it.

Check the workflow's Execution Logs after each one. The logs show exactly which variable came through empty, which saves you guessing.

The true cost of a Formstack and HighLevel setup

Most comparison posts quote the form tool's price and stop. The real monthly cost is the form tool, the CRM, and whatever moves data between them. All prices below are in USD, as listed by each vendor.

ItemPrice (USD)Source, October 2026
Formstack Forms (1 builder user, 25 forms)$83/month billed annually, or $99 month-to-monthformstack.com/pricing
Zapier Professional (if used)From $19.99/month billed annually, 750 tasksZapier pricing blog

Run the numbers on a typical clinic receiving 300 enquiries a month. Via webhook, that is roughly $3 a month in premium executions on top of Formstack and HighLevel. Via Zapier, you are adding a subscription that costs several times more.

Formstack also offers a 14-day free trial with no credit card, per its pricing page. HighLevel lists usage-based charges for SMS, email and phone separately, so budget for those whichever form tool you use. For the full picture, check the official HighLevel pricing page before you commit.

When to skip Formstack and use native HighLevel Forms

I have seen plenty of businesses paying for Formstack purely out of habit. If the form only collects name, email, phone and a couple of questions, native HighLevel Forms or Surveys will do the job for free inside your existing plan.

Keep Formstack when

  • You rely on Formstack Documents or Sign to generate and sign paperwork from submissions.
  • Your compliance team has already reviewed and approved Formstack's data handling.
  • Complex calculations, payment routing or approval workflows live inside the form.

Switch to HighLevel Forms when

  • The form is a straightforward enquiry, booking or lead magnet opt-in.
  • You want attribution, tags and pipelines handled with zero integration upkeep.
  • You are trying to cut subscription costs across the stack.

The decision logic is the same one we used in our Jotform and GoHighLevel integration guide, and it applies to most third-party form tools.

Common mistakes to avoid

  • Sending field labels instead of field IDs, then breaking the mapping when someone rewords a question.
  • Forgetting to enable premium workflow features or rebilling, so client sub-accounts quietly run up agency wallet charges.
  • Mapping to custom fields that were never created, leaving contact records half-empty.
  • Collecting sensitive health or financial details in free-text fields that then sync into every notification email.
  • Testing only one submission and missing duplicate-contact or blank-field errors.
  • Running both the webhook and a Zapier zap on the same form, creating duplicate opportunities.

Frequently asked questions

Does GoHighLevel have a native Formstack integration?

No, there is no direct Formstack app inside HighLevel. You connect the two with a Formstack webhook pointed at HighLevel's Inbound Webhook trigger, or through Zapier or Make using the LeadConnector app. The webhook route is usually the cheapest and fastest.

Is the Inbound Webhook trigger free in HighLevel?

No, it is a premium workflow trigger. HighLevel's support documentation lists a rate of $0.01 USD per execution after 100 free lifetime executions per sub-account. Agencies can rebill this cost to clients with a markup.

Should I send JSON or form data from Formstack?

Choose JSON. It arrives cleanly structured and is easier to read when you fetch the sample request in HighLevel. Also enable field IDs and sub-field names so names and addresses map into separate contact fields.

How much does Formstack cost in 2026?

Per Formstack's pricing page in October 2026, the Forms plan is $83 USD a month billed annually or $99 month-to-month, for one builder user and 25 forms. The Suite plan, which adds Documents and Sign, is listed at $250 USD a month billed annually. Enterprise pricing is quoted on request.

Will duplicate contacts be created if someone submits twice?

Not if you use the Create/Update Contact action with email or phone as the match. HighLevel updates the existing record instead of creating a new one. Test this specifically before going live, because duplicate settings vary by sub-account.

Is Zapier better than a webhook for Formstack?

Zapier is easier for people who already use it and need to send data to several tools. For a single Formstack-to-HighLevel connection, the webhook is faster, cheaper and involves one less vendor handling client data.

Can I trigger a pipeline opportunity from a Formstack submission?

Yes. Add the Create/Update Opportunity action in the same workflow after the contact step. Choose the pipeline and stage, and use If/Else branches to route different services into different pipelines.

When should I replace Formstack with HighLevel Forms?

Replace it when your forms are simple enquiries or opt-ins and you do not rely on Formstack Documents, Sign or complex approval logic. Native forms remove the integration, the premium trigger cost and a separate subscription.
